Membership Form

MEMBERSHIP FEES:

Membership fees are valid from January 1 – December 31.

Individual membership: $15.00
Family Membership: $20.00 (as of January 1, 2026)

Any members enrolling after September 1 will be granted a 16 month membership.

Kindly complete and submit the following form.

NOTE: It is important that the Membership Form and Membership Payment be submitted within one week of each other.

The Membership Coordinator will email you once BOTH your Membership FORM and PAYMENT are received.
